I you are hired, Perry Roofing takes Safety regulations seriously. Perry Roofing requires anyone installing any type of roof in the name of Perry Roofing Contractors to follow the company's safety guidelines and abide by all OSHA rules (see link below) for the duration of employment. We want everyone to work safely and return back to their families. Failure to comply and abide by our safety policies and OSHA standards, it will be grounds for dismissal.

I authorize and release Perry Roofing Contractors to verify all information submitted in support of my / our business application for subcontracting work. I certify that the application and/or information submitted are a complete and accurate description of our business and background. I further certify that the answers to the above questions are true and complete to the best of my knowledge. I understand that any false statements or omissions made by me on this application, or any supplementary or subsequently submitted materials may be grounds for disqualification from any further hiring opportunities at the Perry Roofing Contractors or its affiliated organizations.
